Prescription Refills

As of June 2024, Prescription refills will require appointments, and will no longer be completed without an appointment. Please contact the front desk to book an appointment with your Nurse Practitioner for any prescription refills.
Some appointment concerns can be booked with Registered Nurses. Please contact the front desk to inquire about these appointment types.

Tolerance Policy

Health Care on Yates is committed to providing a safe, secure, and respectful environment for all our patients and staff. 

We have a zero-tolerance policy regarding any form of verbal abuse, displays of aggression, or harassment. This includes yelling, using profanity and/or uttering threats towards our staff.  Any form of abuse, including use of inappropriate words, actions or inactions is unacceptable and will not be tolerated.

Action will be taken, and you will be asked to leave the property and risk losing access to primary care at our clinic.

Our applications are closed as we continue to work through the thousands of requests we have already received for patient care. We are continuing to book intake appointments daily as we work towards our attachment goal of 6800 patients by 2023.
